All patient health care information at Centrum Surgery Center is confidential. Medical records cannot be released to any person or organization without the consent of the patient or the patient’s legally authorized representative, unless authorized by law.
To submit or track a request for medical or billing records, please use the ChartSwap portal.
Every request must be authorized in writing
Requests for the release of protected health information must be submitted in writing and contain all elements required by law. Authorizations must be dated and signed by the patient or the patient’s legally authorized representative.
In the case of a minor, a parent or legal guardian must sign the authorization.

Current Fee Schedule
A current fee schedule for disclosure of health care information is available via the ChartSwap portal.
No-Fee Requests
No fee is assessed when information is sent to another health care provider or for a Social Security disability claim accompanied by a supporting SSA letter.
Patients & Representatives
Patients and patient representatives incur only the fee associated with ChartSwap. All others are invoiced through ChartSwap.
